Initial Primary Consultations for: Weight Loss, Asthma, COPD, Hypertension, Diabetes, Chronic Urinary Tract Infections, Mental Health Consults (depression and anxiety), Hypothyroidism, and many more. The visit will focus on taking a thorough history of the medical issue and previous treatments, labs, medications etc. Visits typically take 30 minutes. Chronic Disease Management: Asthma, COPD, Hypertension, Diabetes, Hypothyroidism, and many more. The visit will focus on strategies for self management at home and reducing exacerbations, troublesome symptoms and hospital admissions. Visits typically take 20-30 minutes. Urgent Care TeleHealth Visits: For simple, problem focused visits like simple Wound Evaluations, Asthma and COPD Exacerbations, Ear Infections, Urinary Tract Infections, COVID symptoms and many more. The visit will be problem focused. Visits typically take 15-20 minutes. Single visit is $50 or plans are available. See plans and pricing for plan options.
Initial Weight Loss Consultations- Visit will focus on goals, education on lifestyle modifications and choosing the right medications for goals. Weight Loss Plans require weekly follow up after initiation of medication for the first month and then monthly visits. Visits typically take 30 minutes for initial consultation and then 15 minutes for monthly follow up. Initial fee includes the initial consult plus, the next 3 weekly visit. Basic TeleHealth visits- Visits can be for any conditions that will only require a single visit or a single monthly follow-up. Conditions can include Hypertension, Birth Control Consults, Erectile Dysfunction, Hair Loss and Menopause symptoms. Weight Loss will be a monthly visit after the first month. Memberships-The Basic Membership covers two visits per month for follow up on any of the above conditions. The Silver Membership will cover 3 visits per month for follow up on the above conditions. Gold Membership covers 4 monthly visits for follow up on the above conditions. Visits for follow up generally take 15-20 minutes. See plans and pricing for options.
* Visits are by text, email, phone and/or video. Fees are due at the time of the visit for initial visits. Memberships are charged on a monthly, recurring basis. Monthly membership visits cannot be carried over and must be used every month. There is no refund for unused visits. Memberships may be cancelled at anytime. Visits must be cancelled with a 2 hour notice to get refunds or to be able to reuse visit. Visits cancelled less than 2 hours prior to scheduled time will NOT be refunded and will not be able to be rescheduled for membership plans. Insurance may be used for any diagnostic or lab tests and medications. These are not included in any plans.
